Tami Leeberg

Founder/Adult and Geriatric Primary Care Nurse Practitioner

As the owner and founder of Senior Solutions Mind & Body, I bring more than two decades of front line experience as an Emergency Room and trauma nurse to the work I do today. After 23 years of caring for patients in their most critical moments, I returned to graduate school and earned my Master of Science in Nursing from Ball State University, completing my Adult & Geriatric Primary Care Nurse Practitioner degree. My career has been shaped by a deep commitment to supporting older adults with compassion, clinical expertise, and a whole person approach to wellness.

With 13 years of experience running a geriatric focused primary care practice, I saw firsthand how many essential services older adults needed but often didn’t receive due to the fast pace and demands of traditional primary care. I founded Senior Solutions Mind & Body to bridge those gaps and provide the dedicated time, attention, and follow through that seniors deserve. My services include Transitional Care Management, Cognitive Function Testing, Annual Wellness Visits, and support for assisted living communities with 1823 forms and other acute care needs. My goal is to deliver thoughtful, comprehensive care that supports both the mind and body as individuals age.

Tami Leeberg

Tami Leeberg is a board-certified Adult and Geriatric Primary Care Nurse Practitioner. Tami is the owner of Senior Solutions Mind & Body, LLC. We are a mobile practice that brings transitional care management and psychiatric care to the comfort of your home. Tami was a registered nurse for 23 years prior to obtaining her practitioner’s degree. While working as a registered nurse, her love was in cardiology, internal and emergency medicine and inpatient psychiatric care. She also was experienced in oncology, neurology, and surgical services. She discovered her love for dementia patients when she became the night charge nurse at Friends Fellowship in Richmond, Indiana. They gave her the opportunity to help develop the original program for the first Alzheimer’s Community in Richmond.

Tami’s desire as a primary care provider is to focus on the geriatric population with a special interest in dementia patients suffering from depression, anxiety and behaviors. Tami received her Bachelor of Science degree from Indiana Wesleyan University and her Master of Science degree from Ball State University. She is currently attending St. Thomas University to be dual board certified as a Psychiatric-Mental Health Nurse Practitioner.

When not working, Tami spends time with her family and friends. She loves being outside, boating, and just relaxing on the beach. She also enjoys traveling, listening to music and being curled up with a good book.

Alex Thorpe

Psychiatric Mental Health Nurse Practitioner

Alex Thorpe, PMHNP- BC is a dedicated psychiatric mental health nurse practitioner with a passion for helping others achieve mental well-being. With extensive expertise in psychiatric pharmacology, Alex focuses on providing compassionate and effective care with individuals in need. His journey is deeply shaped by personal experiences, notably overcoming leukemia, which has inspired his commitment to helping others live fulfilling lives.

This challenge has instilled in him a deep sense of gratitude, purpose, and the importance of cherishing the present. Living on the East Coast of Florida with his wife of 23 years and their three daughters, Alex finds joy in both his personal life and his professional mission. Outside of work, he enjoys being active in his community, reading, running, and playing pickleball. His appreciation for life and learning extends beyond his career; He actively seeks opportunities to integrate his passions into his community involvement, sharing his love for fitness, health, and personal growth.

Guided by a strong sense of altruism, Alex’s approach to life is intentional and driven by a genuine desire to make a meaningful impact. Whether working with patients or connecting with his local community, he is always focused on helping others find balance, happiness, and a sense of purpose.
